TX Black Ops Baseball Liability Waiver and Release

As the parent or legal guardian of a participant in TX Black Ops Baseball (“Participant”), I understand that participation in baseball-related activities is voluntary. I acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in baseball and related athletic activities involves inherent risks, including but not limited to: collisions, overexertion, muscle strains, fractures, heat-related illnesses, and injuries from equipment or field conditions. I voluntarily accept and assume all risks associated with the Participant’s involvement in the program.

2. Medical Authorization

In the event of injury or a medical emergency, I authorize TX Black Ops Baseball staff and representatives to obtain appropriate medical treatment for the Participant. I understand and accept responsibility for any medical costs incurred as a result.

3. General Release and Waiver of Liability

I hereby release, waive, discharge, and hold harmless TX Black Ops Baseball, its coaches, volunteers, owners, sponsors, partners, staff, and affiliated entities from any and all claims, liabilities, damages, losses, or expenses arising from the Participant’s involvement in practices, games, travel, or other team-related activities. This waiver applies to any injury, illness, property damage, or death resulting from participation, regardless of cause, including negligence.

4. Indemnification

I agree to indemnify and defend TX Black Ops Baseball and its representatives against any and all claims, actions, damages, liabilities, or costs (including legal fees) arising out of or related to the Participant’s conduct, participation, or violation of program rules.

5. Insurance

I understand that TX Black Ops Baseball does not provide medical, accident, or personal property insurance for participants. It is my responsibility to secure such coverage independently if desired.

6. Conduct and Compliance

I agree that the Participant will follow all rules and behavior expectations established by TX Black Ops Baseball. I acknowledge that poor sportsmanship, bullying, disrespect toward coaches or officials, or repeated non-compliance may result in suspension or permanent dismissal from the program.

7. Transportation and Supervision

I acknowledge that it is my responsibility to ensure safe and timely transportation to and from TX Black Ops Baseball activities. I understand that the organization is not responsible for participants outside of scheduled event times.

8. Photo/Media Release

I grant TX Black Ops Baseball permission to use photographs, video, or audio recordings of the Participant for marketing, promotion, and social media purposes. I understand no compensation will be provided and all media becomes the property of TX Black Ops Baseball.

9. Acknowledgment

By agreeing to this waiver during online registration, I affirm that I have read, understand, and agree to all terms stated above. I understand that this waiver is intended to be as broad and inclusive as permitted under the laws of the State of Texas, and that if any portion is held invalid, the remainder will still be in full legal effect.
